We understand how locum doctors get paid

A locum doctor’s income rarely comes from one place. Trust shifts through an agency, GP surgery sessions, out-of-hours cover, maybe private clinic work — each engaged differently, each paying differently, and since the public-sector IR35 rules, each potentially taxed differently too.

We act for doctors alongside the dentists, pharmacists and other professionals on our locum & medical side. Limited company accounting is exactly what we specialise in: we’ll keep every engagement reconciled and correctly taxed, and make sure the costs of practising are claimed properly.

    Locum doctor accounting FAQs

Should I work through a limited company as a locum doctor?
It depends on where your work comes from. Trust and agency engagements are often determined inside IR35, while GP surgery cover and private work frequently sit outside — and at locum rates a limited company can be very efficient for that work. Limited companies are all we do: we’ll look at your actual mix and tell you straight whether a company fits, and form it for you when it does (you pay only the £100 Companies House fee).
What about IR35 on NHS engagements?
Since 2017, public-sector bodies determine IR35 status for the engagements they offer, and many trust placements come back inside. That doesn’t make a company pointless — it changes how each engagement is taxed. Our complimentary IR35 service reviews your contracts and working practices so you always know where each engagement stands.
Which expenses can locum doctors claim?
Typically GMC registration, medical indemnity, appraisal and revalidation costs, training and CPD, professional subscriptions such as BMA membership, equipment, and travel between sites where the rules allow. We’ll make sure everything legitimate is claimed and evidenced.
I work for several trusts, an agency and a private clinic — can you handle that?
Yes. Multiple engagement types at different rates, some inside IR35 and some outside, are exactly what locum doctor accounting looks like. We’ll keep every income stream reconciled and each engagement taxed correctly through your limited company.
Can you help me switch from my current accountant?
Yes, and it’s free. Give us your current accountant’s details and we’ll contact them to initiate the transfer and handle it for you.
    What does it cost?
Our core limited company service is £159+VAT per month on a 30-day rolling basis. Some specialist or one-off work may sit outside the monthly fee — we’ll always be clear on any extra costs up front. See full pricing →
